See the http://commons.wikimedia.org/wiki/Image:Wikify.svg. The description page is avaiable, but not the image itself. There is no deletion log avaiable, only upload log. If I go to the restore interface, the software shows two file revisions able to restore. Steps to reproduce: 1) Upload an image to Commons. 2) Edit the image description page on a different wiki. 3) Delete that description page. The image file will be deleted.
Setting highest possible priorities. This endangers all Wikimedia wikis.
Tim fixed the essential bug in r22718, so it won't happen anymore. "Lost" files should be recoverable, just sitting in the wrong wikis' archive directories, so we'll try and work that out pretty soon.

The images which can easily be recovered have been recovered. These images are still on the server, the problem is that finding them would be a difficult and time consuming task. I'll consider requests to recover particular images if the image is important and another copy cannot be found.
Most of the above have been restored now by a search for matching files, except: Prophets_and_Kings.pdf Prophets_&_Kings.pdf 20070604192159!Stop_hand.svg 20061206015143!Wipermail.jpg I'm doing another pass since I think my last check got cut off early.
All four of those files are still missing. :(
The first two files were 'Deleted because "Missing license/source/permission information".' Stop_hand.svg and Wipermail.jpg appear to have been restored. Status is 'RESOLVED FIXED'.
