I'd say some of it is fine as-is, and other bits are good here. The search textinput and button should probably be full-width. Also probably want to make it so the namespace checkboxes will actually show up in a grid (they often don't when there are too many and it looks stupid). The radial things for the type of search seems like an improvement. With the full-width input and button, that way all the buttons are together on one side, the search button is next to the search input, and with the remember checkbox by the bottom, it's by what you'll be wanting to remember, and whatnot. Yes, it's a little weird having the majority of the form potentially below the submit button, but searches are weird.
